  • Full Name: Use the format (Surname, First Name, Middle Name). Ensure it matches your ID.
  • ID No. and Date of Birth: Double-check for accuracy.
  • Gender and Nationality: Fill in correctly.
  • Contact Information: Provide up-to-date phone numbers and email addresses.
  • Disability Details: If applicable, provide clear details about your disability and registration.

3. Academic Qualifications

  • Order Matters: List qualifications starting with the highest (e.g., university, diploma, KCSE).
  • Accurate Years: Indicate the years you attended each institution.
  • Class/Grade/Score: Include your KCSE mean grade, degree classification (e.g., First Class, Second Upper), or relevant scores.

4. Professional Qualifications

  • Specify Professional Areas: Include relevant qualifications such as certifications (e.g., CPA, engineering certifications).
  • Ensure the name of the institution and specialization are clear.

5. Professional Membership

  • List Current Memberships: Include all valid professional memberships.
  • Provide the Membership Number and Date of Renewal for the most up-to-date records.

6. Training and Courses Attended

  • Include training lasting more than two weeks.
  • List the year, course name, and duration. Avoid irrelevant short courses.

7. Employment Details

  • Sign and date the form where required.
